Initial Comments:
An onsite followup survey completed October 1, 2021, found that Keystone Healthcare Services, Llc had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart G, Chapter 601, Home Health Care Agencies. The deficiencies were cited as a result of a State relicensure survey completed July 9, 2021.
